According to new regulations, all health insurance agents and brokers must get written or electronic consent from consumers prior to accessing or updating their marketplace application information.
Before Aventura Insurance Group can continue working on your behalf, you must fill out this form.

STEP 2
Schedule Your Call
Click the button below to schedule your 2025 health insurance call with us.
Whether you received your renewal documents from your provider and would like to keep your current plan as-is, or you want to make changes to your plan, change your provider or explore other health insurance coverage options for the 2025 calendar year, a call must be scheduled.
We kindly ask that you use the link below to schedule your call.
After you have scheduled your call, scroll down to Step 3.

Update Your Information
If the information we have on file for you and all members of your household is current and correct, you can skip this step and you are good to go.
If any changes have been made to your personal information, household and/or income, click the link below to fill out a new client intake form and submit to us prior to your scheduled call.
